Medical Consent

This consent describes the telehealth visit, the prescribing of medication when appropriate, and the ongoing messaging relationship that follows.

§ 01

Telehealth

You understand that the initial review and subsequent visits are conducted remotely. A licensed clinician reviews your intake in writing. Video visits may be scheduled when clinically indicated.

§ 02

Prescribing

When appropriate, your clinician may prescribe medication — including compounded or branded GLP-1 options. The choice of treatment is your clinician's, informed by your history and your stated goals.

§ 03

Messaging

Between visits, you may reach your care team by secure messaging. Responses arrive within one business day in typical cases, faster for urgent clinical questions.

§ 04

Risks & benefits

All treatments have risks as well as benefits. Your clinician will review the relevant ones with you in writing before a plan is finalized. You may decline or discontinue treatment at any time.

Acknowledgement

By continuing with the intake, you confirm that you have read and understood this consent, and that you consent to a telehealth visit with a licensed Nuvora clinician.
